What is pelvic organ prolapse?
The main pelvic organs consist of the bladder, uterus and rectum. Prolapse does not mean these organs are damaged, but refers to a ‘slipping’ down of the organs. When the ligaments and fascial structures that support the pelvic organs become stretched, the organs can sit lower or sometimes outside of the vaginal entrance. This can be caused by pregnancy, childbirth, long-term straining when passing stools, or carrying heavy loads. Symptoms can include vaginal heaviness or dragging, lower abdominal or back pain, difficulty fully emptying your bladder or bowels, recurrent urinary tract infections, or discomfort with intercourse.
Physiotherapy management can be a highly effective way to manage pelvic organ prolapse symptoms. Treatment can include strengthening of pelvic floor muscles, addressing any bladder or bowel issues, ensuring good form and technique during exercise, addressing lifestyle factors and in some cases use of a vaginal pessary.
Pessary Fittings
A vaginal pessary is a silicone support device which sits within the vaginal canal to provide upward support for the pelvic organs (the bladder, uterus, or bowel) or the urethra. For those who experience symptoms of pelvic organ prolapse a pessary can be a great option to both alleviate symptoms and provide upward support for the organs. Pessaries can also be used to provide support and allow the wearer to engage in more high impact exercise and activities, while feeling their pelvic floor is supported. Anyone who is trying to avoid surgery for pelvic organ prolapse may also benefit from trial of a pessary use as an option for conservative management.
The shape and size and use of the pessary is customised to fit your pelvic floor and lifestyle and will vary depending on your symptoms, or your pessary’s purpose. Our physios are specially trained in the fitting and management of pessaries.

Sexual Pain Conditions We Manage

We highly recommend that every woman who gives birth whether it be a vaginal or caesarean birth to have a pelvic physiotherapy check after their six-week GP or obstetrician appointment.
Whether you give birth vaginally or via caesarean our physiotherapists will work with you to address any pelvic floor or musculoskeletal concerns you may have during pregnancy and after giving birth. During this consultation, we will take a thorough pregnancy and birth history, an internal examination if indicated, and will assess your abdominal separation. Whether or not you had any complications during your birth, it is important to check for pelvic floor changes that may have an impact long term. If you had a caesarean birth, there can be an impact on your pelvic floor health after pregnancy and giving birth. If you have had a caesarean birth, we will also assess your scar and give you scar tissue management advice.
You assessment includes personalised advice, guidance, and exercises to fit in with your lifestyle and help guide you safely back to returning to exercise and the activities you love.
